Diagnosis probable

Presentation

Patient walks 5 Km per day, presented with lateral ankle swelling.

Patient Data

Age: 65 years
Gender: Male

Left ankle and foot

x-ray

Soft tissue swelling at the lateral aspect of the ankle.

The peroneus brevis tendon shows diffuse swelling and high T2 signal, wrapping around the peroneus longus tendon. It is surrounded by synovial sheath small effusions, suggestive of peroneus brevis longitudinal split tear on top of chronic tendinitis.

Mild bone marrow edema of the lower end of the fibula.

Osteoarthritis of the second tarsometatarsal joint, showing subchondral edema, narrowed joint space and marginal osteophytes.
