Presentation

Long standing (more than 20 years) left foot swelling

Patient Data

Age: 40 years
Gender: Male

Multiple conglomerates of rounded lesions that demonstrate the "dot in circle sign" on T2 and PD images (hyperintense lesion with hypointense center), involving the mid metatarsals, intermetatarsal, and interdigital soft tissue.

Focal bone erosion in the lateral aspect of the base and part of the shaft of the 2nd proximal phalanx.

Case Discussion

Maduromycosis, also known as Madura foot, is an uncommon musculoskeletal infection caused by multiple fungi or actinomycetes. The patient presented with predominantly soft tissue involvement and minimal bone participation, contrary to the typical presentation, which often features bone destruction and bony remodeling. This case was histopathologically proven to be Madura foot.
