There is a large, well-defined, extra-axial, dural-based mass lesion in the midline along the olfactory groove just above the cribriform plate. The lesion appears isointense to gray matter on T1WI and T2WI with homogenous avid post-contrast enhancement. Perilesional mild edema is noted. Inferiorly the lesion is compressing the pituitary gland. Superiorly it is compressing the optic chiasm. Laterally it encases the internal carotid arteries in the cavernous sinuses.
Case Discussion
The features represent olfactory groove large meningioma.
