Meningioma with hyperostosis and sunburst periosteal reaction

Diagnosis probable

Presentation

4-year history of a scalp mass. No history of trauma. On examination, the mass is soft, non-tender, non-fluctuant and translucent.

Patient Data

Age: 20 years
Gender: Male

There is a large, extra-axial, well-circumscribed, vividly-enhancing hyperdense mass seen along the falx cerebri, involving the occipital and bilateral parietal regions. The mass does not extend below the tentorium cerebelli. A dural tail is noted. There is the homogeneous enhancement of contrast. No focal calcifications were seen within.

Obliteration of the posterior superior sagittal sinus with thrombus within the residual superior sagittal sinus adjacent to the mass.

“Sunburst" periosteal appearance is seen involving the occipital bone and the bilateral parietal bones.

Case Discussion

Findings are highly suggestive of a meningioma. Differential diagnoses of:

  1. osteosarcoma

  2. Ewing sarcoma

  3. solitary fibrous tumor of the dura
