Methanol poisoning - chronic putaminal necrosis

Presentation

Chronic alcohol abuse. History of methanol poisoning. Spastic tetraparesis for a year.

Patient Data

Age: 60 years
Gender: Male

MR images reveal bilateral putamen necrosis evident by a high signal on T2WI, FLAIR, and ADC and a low signal on DWI and T2* GRE. Additionally, signs of demyelination of the corpus callosum are visible on sagittal FLAIR sequences as hypointense lesions.

Case Discussion

Bilateral putamen necrosis is quite a specific finding in patients with methanol intoxication. The presented patient has a history of alcohol abuse and methanol poisoning. Spastic tetraparesis developed just after acute intoxication. The patient stayed in such a state for a year and eventually, an MRI was performed.
