Presentation

History of trauma to the left wrist during military training 10 years ago, presented with loss of function of the left wrist.

Patient Data

Age: 35 years
Gender: Male

Volar dislocation of the lunate and capitate with loss of radiolunate articulation. Neither the capitate nor lunate is co-linear with the long axis of the radius. Fracture of the triquetrum. Pseudo articulation of the lunate with the palmar aspect of the distal radius and associated osteoarthritic changes. Diffuse sclerosis of the scaphoid with subarticular cystic changes. The constellation of findings is consistent with poorly managed chronic midcarpal dislocation - stage III Mayfield classification of carpal instability, with osteoarthritis of the wrist joint.

Disruption of the scapholunate and lunotriquetral ligaments. Triangular fibrocartilage degenerative tear. Significant loss of hyaline cartilage of radiocarpal joint with significant joint space narrowing. Marrow signal changes of the scaphoid bone with subarticular cyst formation in the scaphoid and distal radius.

Midcarpal dislocation represents dislocation of the lunate and capitate with fracture of the triquetrum or lunotriquetral ligament disruption, according to the four stage process of carpal or perilunate instability described by Mayfield.
