Misplaced chest tube

Presentation

SOB, productive cough, and on/off fever for a few months.

Patient Data

Age: 70 years
Gender: Male

Chest tube for drainage of large right pulmonary lesion which is showing air-fluid level is placed through inter-costal space into the thoracic cavity and its tip appears near to right hilar region exterior to the above-mentioned lesion likely placed in fault as considered lung abscess.

A definable hypodense lesion with internal thin septa is noted in the right lobe which is likely suggestive of a hydatid cyst.

Surgical emphysema is seen in the subcutaneous tissues of the right chest wall.

Case Discussion

CT findings are of a misplaced chest tube in the right hemithorax as described above.

The lesions in the right lung and right lobe of the liver are most likely suggestive of hydatid cysts of which the pulmonary one is ruptured/infected.
