There is a large subcutaneous collection involving the anterior aspect of the knee and the distal femur. This collection elicits a low signal on T1-weighted images and high signal on fluid-sensitive images and contains characteristic globules of fat. This collection measures about 14x2 cm in its transverse diameter and 9 cm in its craniocaudal diameter.

Case Discussion

Findings are consistent with Morel-Lavallee lesion. Morel-Lavallee lesion is a closed traumatic degloving injury located at the interface between the subcutaneous fat and the fascia, most commonly overlying the hip and buttock . The prepatellar region is the second most common location. The MRI appearance is variable, ranging from a simple unilocular fluid collection to heterogeneous collections containing blood products and fatty debris.
