Multiple bilateral hyperintensities in the periventricular white matter and juxtacortical white matter of frontoparietal lobes, posterior limb of the right internal capsule, left lentiform nucleus, in the left middle cerebellar peduncle and left cerebellar hemisphere  Few of these lesions appear hyperintense on DWI images suggesting active lesions.

Hyperintensity and edema involving intraconal segment of right optic nerve root consistent with right optic neuritis.

Case Discussion

Overall the MRI findings are suggestive of multiple sclerosis with right optic neuritis. There are characteristic periventricular, juxtacortical, and infratentorial lesions in favor of the diagnosis.
