Multiple system atrophy

Atrophy and thinning of the pons, medulla and the middle cerebellar peduncles with prominence of the prepontine cisterns and the fourth ventricle. A cruciform T2 hyperintensity is seen in the atrophied pons displaying the so called hot cross bun sign.
There is also marked atrophy of the cerebellum involving the cerebellar hemispheres and the vermis.
Global cortical atrophy is also noted.
Case Discussion
MRI findings of atrophy of the pons, medulla, middle cerebellar peduncles and the hot cross bun sign suggest multiple system atrophy.
