MRI demonstrates atrophy of the brainstem, cerebellum, and symmetric atrophy of middle cerebellar peduncles.
There are also cruciform hyperintensities in the pons "Hot cross bun sign".
White matter hyperintensities in both cerebral hemispheres suggest chronic vessel ischemia.
Case Discussion
MRI features are consistent with multiple system atrophy - cerebellar subtype (MSA-C) that demonstrated the hot cross bun sign.
