Presentation

Pain, swelling in bilateral lower limbs and inability to walk properly

Patient Data

Age: 7 years
Gender: Male
mri

Diffuse extensive edema involving bilateral gluteal muscles, muscles of the thigh, visualized adductor muscles and visualized muscles around the knee joint with relative sparing of muscles of the posterior compartment of the thigh. Subcutaneous and fascial edema is also seen.

No obvious intramuscular collection is seen in the present scan.

No obvious fatty infiltration of visualized muscles is seen in the present scan.

Bilateral femurs appear normal in signal intensity with no obvious erosions or marrow edema.

Case Discussion

Overall above findings suggest the possibility of myositis. This was later confirmed to be juvenile dermatomyositis on laboratory work up.
