Presentation

Disseminated tuberculosis with left lower limb paralysis and painful swelling in the left thigh region.

Patient Data

Age: 25 years
Gender: Male

Soft tissue calcification around the left proximal femoral shaft, however, the adjacent bone appears normal.

Soft tissue lesion in the left proximal thigh region in the anterior aspect having peripheral ossifications. The underlying bone appears normal.

Linear undisplaced fracture of sacral ala on the right.

MRI Pelvis

Soft tissue hetero-intense lesion in the left proximal thigh region in the anterior aspect having peripheral ossifications and marked edema in the left iliacus, gluteus, and vastus group of muscles. Few small areas of the peripheral rim enhancement within the lesion.

Case Discussion

Imaging findings are suggestive of myositis ossificans.

Most cases of myositis ossificans occur as a result of trauma. Another group that is especially prone to myositis ossificans are paraplegics, usually without evidence of trauma as in this case.
