Presentation

Upper respiratory tract infection (URI) and headache for about one week

Age: 30 years

Intra-axial focal abnormal CSF density lesion measured 12 x 6 mm was seen in the right frontal white matter

A small well-defined subcortical unilocular cystic lesion of the right frontal lobe measuring 12 x 6 x 10 mm, elicits low signal intensity on T1 and FLAIR, and high signal on T2 with no restricted diffusion (CSF signal). No peripheral enhancement or surrounding edema or gliosis.

Case Discussion

This is an incidental small cystic lesion highly consistent with a neuroglial cyst. A cerebral hydatid cyst is a plausible differential diagnosis radiologically, but it is usually perfectly spherical. An ependymal cyst is another differential diagnosis but is typically periventricular in location. An enlarged perivascular space could be considered but they are often multiple and cluster around the basal ganglia.
