Presentation

Myasthenia.

Patient Data

Age: 45 years
Gender: Male

Well-defined left-sided mass with the hilar structures visible through the mass "hilum overlay" sign of anterior location on the lateral scout view.

Well-circumscribed heterogeneously enhancing soft tissue mass of anterior mediastinal location, lateralized to the left with no calcification or fatty component within. The lesion is abutting the costochondral cartilage anteriorly and ascending aorta and pulmonary trunk posteriorly.

No mediastinal or hilar lymphadenopathy.

No pleural or pericardial effusion.

No obvious bony erosion or chest wall invasion.

The lung window shows bilateral ground‐glass opacities of peripheral distribution mainly of the apical segment of the right lower lobe and the posterior segment of the left upper lobe.

Homogeneous hepatosplenomegaly.

Case Discussion

CT features of a well-circumscribed heterogeneously enhancing soft tissue mass of the anterior mediastinum in a middle age patient with myasthenia most consistent with a thymoma that was confirmed by an ultrasound-guided biopsy as a non-invasive thymoma.

COVID-19 pneumonia was an incidental finding confirmed by a positive RT-PCR test.
