Presentation

Non-specific knee discomfort without recent trauma. Incidental finding.

Patient Data

Age: 14 years
Gender: Female
x-ray

2.5 cm metaphyseal lesion arising from the cortex and extending into the medullary
cavity with a sclerotic narrow zone of transition. No periostitis. Appearances
consistent with a non-ossifying fibroma.

Case Discussion

Non-ossifying fibromas (NOFs) are benign lesions commonly seen in children and usually of no clinical significance. Occasionally very large non-ossifying fibromas may require prophylactic curettage due to risk of a pathological fracture

Often - as here - the appearance is so typical that no real differential diagnosis will be entertained.

NOFs are one of the "Don't touch" lesions.
