Case contributed by Bahman Rasuli
Diagnosis almost certain

Presentation

Chronic pain in the left ankle and leg.

Patient Data

Age: 14 years
Gender: Male
x-ray

X-ray demonstrates a well-circumscribed lytic bony lesion in the anterolateral aspect of the distal diaphysis of the left tibia with a well-demarcated sclerotic margin associated with a focal thinning of the adjacent anterior cortex.

No periosteal reaction is present.

It does not abut the growth plate.

Pes cavus is noted.

Case Discussion

The imaging results suggest the presence of a benign non-ossifying fibroma. Non-ossifying fibromas are typically observed in adolescents and young adults and are commonly found in the femur and tibia. They are located in the dia-metaphyseal region and appear as a well-defined lucent lesion with a sclerotic margin, demonstrating mild eccentric expansion. Cortical thinning is observed, but there is no evidence of cortical destruction.
