Occult femoral neck fracture

Presentation

Left hip pain after a fall.

Patient Data

Age: 60 years
Gender: Female
x-ray

Normal.

No fracture.

Femoral neck marow edema suggestive of occult hip fracture.

x-ray

Left dynamic hip screw appropriately positioned without complication, otherwise normal.

Case Discussion

Femoral neck fractures that are invisible on plain radiographs and CT are called "occult" and should be suspected in elderly osteoporotic patients who present with hip pain and refuse to bear weight after a fall. In these cases, a diagnosis of femoral neck fracture can only be made with MRI.
