Diagnosis probable

Presentation

Severe vision loss. No pain or significant medical history.

Patient Data

Age: 30 years
Gender: Male

Mild hyperdensity of the right optic nerve sheath with avid focal enhancement after contrast injection.

No intracranial bleeding or hydrocephalus.

No brain herniation.

The ventricular system is unremarkable.

Focal thickening and homogeneous enhancement of the right optic nerve sheath. This thickening is T2 hyperintense and T1 iso-intense compared to the optic nerve.

Case Discussion

The main differential diagnosis discussed for this patient was optic perineuritis, but the lesion was stable over 6 months of evolution and did not respond initially to corticosteroid therapy.
