Case contributed by Dalia Ibrahim
Diagnosis certain

Presentation

Left proptosis.

Patient Data

Age: 40 years
Gender: Male

Left intra-orbital intraconal well defined mass lesion. It elicits low signal on T1, high signal on T2 and STIR WI. On the early arterial contrast enhancement, it shows peripheral nodular contrast enhancement, with delayed homogeneous contrast enhancement of the lesion. The lesion displaces the optic nerve superiorly. It's smoothly abutting the adjacent extra-ocular muscles. Left proptosis is noted.

Case Discussion

Pathologically proven case of orbital cavernous hemangioma.

The case illustrates the typical MRI features of orbital cavernous hemangioma, especially the gradual progressive contrast enhancement on sequential post contrast series.

Cavernous venous malformations of the orbit, also known as cavernous hemangiomas, are the most common vascular lesion of the orbit in adults.
