Presentation

Facial trauma and consequent limitation of movement of the right eye.

Patient Data

Age: 40 years
Gender: Male

Right orbital floor blow-out fracture with fat and inferior rectus muscle hernia within the fracture gap and both sides preseptal and lacrimal gland soft tissue swelling are seen. Hemorrhage within the fracture gap and related orbital fat, and inferior rectus muscle swelling are noted.
Fluid and thick mucosa in paranasal sinuses more in the ethmoid and maxillary sinus and retention cyst in left maxillary sinus are seen.

Case Discussion

The case illustrates the non-contrast MDCT features of orbital floor blow-out fracture and inferior rectus muscle entrapment within the fracture gap. One of the major complications of orbital floor fracture is entrapment and dysfunction of the inferior rectus muscle and as a result upward gaze limitation of the affected eye.
