Orbital roof fracture and intracerebral hemorrhage

Presentation

Facial and head trauma and right side eyelids swelling and confusion.

Patient Data

Age: 80 years
Gender: Male

Displaced fracture in the right orbit roof extended within the ethmoid roof and upper medial wall of the orbit and also in the lateral wall of the orbit is seen. Large cerebral hemorrhagic contusion in right side frontal lobe adjacent to right orbit roof with marginal white matter edema and mass effect on related parenchyma is seen. Eyes have no native lens and small calcified foci in medial and lateral rectus muscles insertion to eye globes are seen. Bilateral proptosis and partial empty sella are also evident.

Case Discussion

A cerebral hemorrhagic contusion is one of the major complications of orbital roof fractures which increases the morbidity rate of the patient. The cerebral hemorrhagic contusion is usually seen in the frontal lobe anterior inferior pole and temporal lobe anterior pole.
