Osteochondritis dissecans - knee

Diagnosis almost certain

Presentation

Locking of the left knee.

Patient Data

Age: 35 years
Gender: Male

Non-displaced osteochondral lesion of the medial femoral condyle. The osteochondral fragment is of low signal on T1 and isosignal to the normal bone on PD fat sat with surrounding high signal line "rim sign" and subchondral edema.

Mild joint effusion is seen.

ACL, PCL, and collateral ligaments are intact. No meniscal tear is seen.

Case Discussion

The MRI features are most consistent with an osteochondritis dissecans of the knee, stage III according to the osteochondral injury staging.
