Pain and swelling of the right great toe.
Patient Data
Osteolysis and destruction of the phalanges, head, and neck of the first metatarsal bone noted with adjacent extensive soft tissue swelling - suggestive of osteomyelitis.
An old linear fracture of the head of the second metatarsal bone was noted.
Sclerosis with subchondral flattening of the head of the 3rd metatarsal bone noted - suggestive of Freiberg's disease.
Case Discussion
Osteomyelitis is common in diabetic patients, especially those with uncontrolled diabetes, who have soft tissue cellulitis and skin ulcerations.
In non-diabetic patients, it is seen following penetrating injury or secondary to hematogenous spread in generalized infection.
