Ostitis condensans ilii and osteitis pubis

Presentation

Low back pain.

Patient Data

Age: 40 years
Gender: Female

Bilaterally, the iliac side of the sacroiliac (SI) joint demonstrates triangular sclerosis, suggesting osteitis condensans ilii.

The symphysis pubis shows subchondral erosive changes, joint irregularity, and sclerosis, partially obscured due to the sacrum on the frontal projection.

Case Discussion

Osteitis condensans ilii is a benign condition typically found in women of childbearing age and characterized by bilateral, symmetrical, and non-inflammatory changes in the iliac bones adjacent to the sacroiliac joint.

Osteitis pubis is defined as a non-infectious, inflammatory condition of the pubic symphysis that may also occur due to pregnancy and childbirth amongst other traumatic, post-surgical or arthritic causes.

Neither are a specific cause for low back pain.
