Diagnosis almost certain

Presentation

Bilateral earache for a few days.

Patient Data

Age: 10 years
Gender: Male
ct

There are fluid density areas involving Prussak spaces of bilateral middle ear cavities and around the ossicle chains; however, no definite bony erosion is seen.

Bilateral internal and external ears, along with mastoid complexes, are within normal limits.

Partial opacification of the left anterior ethmoid air cells is noted.

The rest of the imaged paranasal sinuses appear unremarkable.

Case Discussion

The current CT findings and clinical presentation of the patient are more in favor of bilateral otitis media rather than cholesteatoma. Visualization of fluid in the Prussak space is the first sign of an infective/inflammatory process, as described above.
