Ovarian dermoid cyst

Presentation

Pelvic pain with a complex mass on ultrasound exam (not shown).

Patient Data

Age: 35 years
Gender: Female
mri

Well-defined complex cystic mass arising from the left ovary with two components, the main component is of high signal on T1 and T2, attenuated on fat-saturated sequences (fatty component) and floating ball of intermediate signal on both T1 and T2 sequences (floating ball sign). Mild peripheral enhancement seen on postcontrast sequences.

Anteverted retroflexed uterus, the thickness of the transition zone = 9 mm.

Right ovary is normal in size with a corpus luteum and a small hemorrhagic follicle.

Case Discussion

MRI features of an ovarian dermoid cyst with intracystic floating ball sign which is uncommon but pathognomonic for ovarian dermoid cyst.
