Ovarian serous cystadenoma

Presentation

Right flank pain for 3 weeks and microscopic hematuria. History of simple right adnexal cyst.

Patient Data

Age: 50 years
Gender: Female
ct
  • no radiopaque calculi are seen in the kidneys, ureters and urinary bladder; no hydronephroureter is seen on either side; tiny opacities in the right hemi pelvis which are outside the line of the distal ureter and are likely phleboliths 

  • enlarged liver measuring approximately 21 cm and spleen measuring 13 cm

  • large well-defined simple fluid density cystic structure measuring 7.0 x 8.0 cm in the right iliac fossa. No calcifications are seen in it

ultrasound
  • anechoic right adnexal/para-ovarian cyst measuring 7.0 x 8.6 cm

  • no septations, internal soft tissue components or internal vascularity

mri
  • large unilocular cyst measuring 9 cm seen in the right adnexa, which is likely an exophytic cyst arising from the right ovary;  no septations or solid component; no evidence of invasion into the surrounding structures; no diffusion restriction or abnormal enhancement is seen in it

  • normal left ovary

  • bulky uterus

  • no free fluid is seen

  • no significant lymphadenopathy

Histopathology of the excised right ovarian cyst showed serous cystadenoma.

Case Discussion

Imaging features are suggestive of a benign right ovarian cyst, like serous cystadenoma which was later confirmed histopathologically.
