Presentation

Right lower abdominal pain for three days duration

Patient Data

Age: 25 years
Gender: Female
ultrasound

Gravid uterus.

The right ovary appears hyperechoic, enlarged in volume, and midline in location, without convenient internal vascularity, suggesting ovarian torsion.

The left ovary appears grossly unremarkable, with preserved vascularity at the time of the exam.

Mild pelvis-free fluid collection.

Case Discussion

Ovarian torsion is a gynecological emergency that requires early diagnosis to preserve ovarian function.

Abnormal ovarian vascularity, increase ovarian volume , increase echogenicity and the presence of pelvic free fluid were the most sonographic signs for ovarian torsion.

On laparoscopic evaluation the right ovary was twisted multiple turns around the pedicle.
