Ovarian torsion

Presentation

Acute onset abdominal pain

Patient Data

Age: 15 years
Gender: Female
mri

The uterus is anteverted and normal in size.

The right ovary is diffusely bulky with midline in location with follicles within, without showing post-contrast enhancement. Right ovary volume- 47 cc

A cystic lesion superior to the urinary bladder showing T2 hyperintense, T1 hypointnse signal with peripheral rim enhancement. No evidence of solid component or septation. Few follicles are noted in the periphery of the cyst.

The left ovary is noted posterior to the uterus and shows multiple follicles within and normal post-contrast enhancement.

No e/o free fluid in the pelvis.

Findings are suggestive of right ovarian torsion with right ovarian cyst

Case Discussion

The patient underwent emergency exploratory laparotomy with right ovarian cyst puncture and right ovarian oophoropexy.
