Ovarian torsion - pediatric

Diagnosis almost certain

Presentation

Severe right pelvic pain.

Patient Data

Age: 5 years
Gender: Female
ultrasound

The right ovary is heterogenous, markedly enlarged for her age in which it measures about 2.5 x 2.5 x 2.9 cm (10 mL in volume) while the left ovary measures about 0.5 mL in volume; showing echogenic stroma with multiple peripherally displaced follicles, some of them show follicular ring sign.

Color Doppler shows no obvious internal vascularity.

Minimal free fluid is noted surrounding the right ovary.

The appendix is visualized, and it appears unremarkable.

Case Discussion

The diagnosis of ovarian torsion was confirmed surgically and right side detorsion with bilateral oophoropexy was performed.
