Presentation

History of cerebral palsy, epilepsy, failure to thrive and blindness. On gastrostomy feeding tube.

Patient Data

Age: 20 years
Gender: Female
x-ray

A gastrostomy tube is seen in place. Marked scoliosis in the visualized spine. Diffuse osteopenia in the visualized bony skeleton. Neglected right DDH.

x-ray

Gastrostomy tube seen on the previous study had been removed. An NGT is seen in place. A metallic density, is seen projecting over the gastric bubble, which is an over-the-scope clip (OTSC).  Non-specific bowel gas pattern.

CT pulmonary angiogram

A metallic device is seen along the anterior wall of the stomach, which is an over-the-scope clip (OTSC). An NGT is noted in place. No CT evidence of pulmonary embolism. Scattered patchy consolidative changes in both lungs, particularly more marked in the right lower lobe. Mild right and minimal left pleural effusions. No pericardial effusion, significant mediastinal lymphadenopathy, pneumothorax or pneumomediastinum. Marked scoliosis noted in the visualized spine.

Case Discussion

The patient developed a gastro-cutaneous fistula due to infection of the gastrostomy tube tract. This fistula was managed with an endoscopic over-the-scope clip (OTSC).
