Park’s classification is the most often used surgical classification for differentiating between four types of fistula. The course of the fistula and its relationship to the anal sphincters is described in the coronal plane:
intersphincteric: fistula crosses intersphincteric space and does not cross external sphincter
transsphincteric: fistula crosses from internal sphincter to external sphincter through intersphincteric space, and then to ischioanal fossa
suprasphincteric: fistula crosses into intersphincteric space, and over puborectalis muscle through levator ani muscle into ischiorectal fossa then perineal skin
extrasphincteric: fistula crosses from perineal skin through ischiorectal fossa and levator ani muscle into rectum
