Pathologic lesser trochanter avulsion fracture

Presentation

Right hip pain, history of malignancy.

Patient Data

Age: 55 years
Gender: Male

Avulsion fracture of the right lesser trochanter with proximal displacement of the lesser trochanter. Destruction of the lesser trochanter and adjacent femur.

Confirms avulsion fracture of the right lesser trochanter with proximal displacement and destruction of the lesser trochanter and adjacent femur. Multiple other lytic lesions also present.

Post dynamic hip screw placement appropriately positioned without complication.

Case Discussion

The bone destruction in this patient indicates that this is a pathologic fracture, in this case due to metastatic disease in this patient with an underlying malignancy. Multiple other lytic lesions are best appreciated on the CT scan. The lesser trochanter is displaced proximally by the pull of the iliopsoas tendon/muscle.
