Presentation

The patient manually bent his penis to achieve detumescence.

Patient Data

Age: 20 years
Gender: Male
ultrasound

Features consistent with a penile fracture, with disrupted tunica albuginea identified at the base of the penis (proximal third), ventral surface, right-sided. The defect measures approximately 5.0 mm in width and 4.5 mm in AP diameter.

There is regional active venous hemorrhage demonstrated by a serpiginous vein at the site of the defect with progressive and significant hematoma extending from the base of the penis to the glans eccentrically right-sided. There is a significant mass effect and consequent eccentric left-sided displacement of bilateral corpora cavernosa and corpora spongiosum.

The penile septum, urethra, left sided corpora cavernosum and corpora spongiosum are intact.

There is a focal hematoma involving the right sided corpora cavernosum additionally.

Case Discussion

Features consistent with a right-sided grade II penile fracture 1 with a significant and large hematoma and active venous hemorrhage during ultrasound assessment and consequent progressive swelling, bruising and clinically evident aubergine sign.
