Penile implant and artificial urethral sphincter

Presentation

Artificial urethral sphincter and penile prosthesis with sphincter not working. Query displaced sphincter cuff. Previous radical prostatectomy and salvage radiotherapy.

Patient Data

Age: 60 years
Gender: Male

Artificial urethral sphincter located around the spongy urethra, with intact tubing, left hemiscrotal pump and a reservoir in the left abdominal wall. The hemiscrotal pump appears deflated. IDC in situ.

Bilateral cavernosal implants appear well positioned with intact tubing, right hemiscrotal pump and right abdominal wall reservoir. The abdominal wall reservoir has a similar appearance to the prior CT.

Pelvic structures are unremarkable. No free fluid.

Impression

Possible deflation of the left hemiscrotal artificial urethral sphincter pump. Tubing appears intact.

Case Discussion

The patient underwent cystoscopy and assessment of the AUS. The cuff was normally located but had ulcerated the urethral mucosa at the level of the cuff. The AUS was tested and worked well. No definite cause was found for dysfunction.

Several days later the patient represented in urinary retention and underwent suprapubic catheter insertion.
