Presentation

The patient reported pain in the left lower back teeth region for the past 2 months. The pain is continuous, dull, and exacerbated while eating.

Expansile lucency with thinning of buccal and lingual cortical plate involving the left mandible symphysis/body and apices of 34 and 35.

pathology

The hematoxylin and eosin stained histopathological section of the given specimen shows no evidence of lining epithelium and exhibits only connective tissue stroma. The stroma is moderately collagenized with diffuse chronic inflammatory cell infiltrate, predominantly lymphocytes. Deeper connective tissue shows normal appearing peripheral bone. Numerous endothelial-lined blood vessels and extraverted RBCs are noted.

Case Discussion

Periapical cysts, also known as radicular cysts, stand as the most common odontogenic cysts in the jaw. Originating from cell rests of Malassez in response to periapical inflammation, these cysts progress through three stages: initiation, cavity formation, and enlargement via osmosis. Surgical intervention is the preferred treatment to prevent recurrences.
