Presentation

Small soft swelling on palpation at the upper pole of the left testicle.

Patient Data

Age: 45 years
Gender: Male
ultrasound

Homogeneous hyperechoic lesion, without flow signals, round and well circumscribed, approximately 5 mm, delimited in the paratesticular space, just anterior to the upper pole of the left testicle from which it appears split.

Testicles and epididymis are normal. No evidence of inguinal lymphadenopathy.

Removal surgery

Histological report

Well-defined capsulated lesion composed of lobules of mature fatty tissue correlated with lipoma.

Case Discussion

Peritesticular lipoma is the most frequent benign tumor of the spermatic cord and paratesticular tissues with a very low incidence rate. Lipomas are without vascularization. In US, lipomas are typically hyperechoic but sometimes may occasionally be hypoechoic because these tumors can contain fibrous, myxoid, and vascular tissue.
