Presentation

Left eyelid weakness.

Patient Data

Age: 75 years
Gender: Female

A well-defined hyperdense sellar lesion with suprasellar extension is noted.

Capacious CSF spaces compatible with the patient's age.

mri

A well-defined sellar lesion with suprasellar extension is noted, exhibiting heterogeneous enhancement on post-contrast images and invading the left cavernous sinus above the extracavernous portion of the internal carotid artery. These features are indicative of a pituitary macroadenoma, classified as grade 3A according to the Knosp classification for cavernous sinus invasion.

Case Discussion

Pituitary adenomas are benign, slow-growing tumors. They may be discovered incidentally but mostly present with mass effect upon the optic chiasm. Additionally, they may invade the cavernous sinus. Large tumors may result in hydrocephalus.

Unfortunately histological proof is not available.
