Pituitary macroadenoma with apoplexy

Presentation

Drooping of left eyelid, history of delivery 15 days back

Patient Data

Age: 30 years
Gender: Female

Lobulated sellar, suprasellar lesion is seen causing enlargement of sella with erosion of dorsum sella. It is mildly splaying optic chiasma superiorly. It is abutting cavernous segments of bilateral internal carotid arteries with no obvious extension into cavernous sinuses in
present scan.

It appears hyperintense on T1, heterogeneously hyperintense on T2W images (suggesting hemorrhage) and shows mild, predominantly peripheral enhancement on post contrast scans.

Case Discussion

Findings are suggestive of pituitary macroadenoma with apoplexy and explain the patient's symptoms.
