Presentation

Retired builder. Shortness of breath. No fever.

Patient Data

Age: 70 years
Gender: Male
x-ray

Large left sided pleural effusion with mild mediastinal shift.

Circumferential pleural thickening in the left upper hemithorax.

Right lung clear.

Large left-sided pleural effusion with multiple pleural soft tissue nodules, the largest, 22mm lies adjacent to the descending thoracic aorta. Pleural thickening along the mediastinal surface in the upper left hemithorax. Calcified pleural plaques.

Right lung clear.

No mediastinal lymphadenopathy.

Case Discussion

This case has all the clinical and radiological features indicative of mesothelioma.

A prior profession in which asbestos exposure is common.  Pleural plaques confirming prior asbestos exposure and then a malignant effusion with pleural thickening extending on the mediastinal surface.

This was confirmed histologically.
