Pontine cavernous venous malformation

Presentation

Free past medical history, presented with recurrent headaches not improving by analgesia

Patient Data

Age: 25 years
Gender: Female

Non-enhancing well defined multi-lobulated lesion (giving pop-corn appearance) seen in posterior aspect of pons, showing blooming artifact most obvious at GRE sequence, associated with a rim of surrounding hyperintense signal on both T1 and T2WI with diffusion restriction as well as a dependent level of blooming seen in lateral ventricles suggesting blood products.

Overall features are suggestive of pontine cavernous venous malformation with intraventricular leak.

Case Discussion

Cavernous venous malformations are vascular anomalies, can be asymptomatic, or may cause symptoms due to mass effect or hemorrhage, which indicates surgical removal.

Caution must be taken at resection if there is an associated developmental venous anomaly, due to risk of causing intractable bleeding.

T1 and T2 signal intensities may vary according to age of blood in cavernous malformation.
