Presentation

Chronic pain in both feet.

Patient Data

Age: 40 years
Gender: Female
x-ray

Bilateral polydactyly of the fifth toe with partial duplication of the distal third of the fifth metatarsal shaft and complete phalangeal duplication.

Case Discussion

Polydactyly is the most common congenital anomaly of the hands and feet, mostly autosomal dominant and the most common of the forefoot. Polydactyly of the foot is present in 1.7 cases per 1,000 live births. It can present as an isolated anomaly or associated with other syndromes 1-3.

Being divided into three main categories 1:

  • preaxial: affects the radial/tibial region

  • axial/medial: central region of the hand or foot

  • postaxial: affects the ulnar/peroneal region

Postaxial is the most common and axial is the least common.

It can be classified by the configuration of the metatarsal, according to Venn Watson 4:

  • normal metatarsal with distal phalanx duplication

  • block metatarsal

  • Y-shaped metatarsal

  • T-shaped metatarsal

  • normal metatarsal shaft with a wide head

  • duplicated ray
