Presentation

4-day history of headache and blurry vision. Long-standing hypertension and diabetes.

Patient Data

Age: 70 years
Gender: Male

Brain MRI w/o Contrast

Large region of restricted diffusion throughout the right occipital lobe and hippocampus compatible with acute ischemic infarct of the right posterior cerebral artery territory.

Patchy T2 shine in the left corona radiata may reflect subacute ischemia.

Extensive T2/FLAIR white matter hyperintensities throughout a subcortical and periventricular white matter distribution as well as throughout the left midbrain and pons which may reflect sequelae of chronic microvascular ischemic changes.

Case Discussion

This represents a case of a posterior cerebral artery infarct.

The treatment plan included dual antiplatelet therapy, statin, and insulin therapy.
