Prefemoral fat pad impingement syndrome

Diagnosis almost certain

Presentation

Anterosuperior knee pain

Patient Data

Age: 25 years

Significant edema and hyperintensity is seen in lateral aspect of posterior suprapatellar fat pad.

Insall Salvati ratio is approx. 1.15 (Normal), essentially ruling out patella alta.

Mild proximal patellar tendinosis is seen along attachment site at inferior pole of patella.

Mild edema is seen in Hoffa’s fat pad along infrapatellar plica, possibly infrapatellar plica injury.

Possible low grade sprain of mid segment, femoral attachment of anterior cruciate ligament is seen. Its continuity is maintained.

Linear hyperintensity seen in posterior horn medial meniscus not intersecting with articular margins (Grade 2 signal).

Case Discussion

Significant edema and hyperintensity is seen in lateral aspect of posterior suprapatellar fat pad consistent with prefemoral / posterior suprapatellar fat pad impingement syndrome. This is the likely cause of pain.
