A large pedunculated mass is seen arising from the anterior lower endometrial wall, filling the lower segment of uterine cavity and prolapsing through a distended endocervical canal into the left lateral fornix of the vagina. It is of inhomogeneous high signal on T2WI and intermediate signal intensity on T1WI FS with intense enhancement following IV contrast. No diffusion restriction.

Mild fluid is seen in the Douglas pouch.
