Pulmonary sequestration - chronic

Presentation

Chronic cough.

Patient Data

Age: 70 years
Gender: Female

Triangular opacity in the medial mid to lower zone on the left with internal lucency.  No further focal opacity. No pleural effusion. 

There is focal opacity within the medial basal segment of the left lower lobe with tubular internal lucencies and coarse calcifications. No convincing connection to the left lower lobe bronchi and there is a possible arterial supply from the thoracic aorta.

Poor arterial enhancement on this CTPA study, however, systemic arterial supply of the medial basal segment left lower lobe is seen, arising from the celiac axis. 

Case Discussion

Chronic sequestration is uncommon. This appears to be an intralobar sequestration with pulmonary venous return appearing to be via the left inferior pulmonary vein. The celiac axis is an uncommon site for the arterial supply for sequestration.
