Pyonephrosis

Diagnosis almost certain

Presentation

Left flank pain for last 6 months. Associated with high grade fever and vomiting for last one week.

Patient Data

Age: 35 years
Gender: Female
ultrasound

Abdominal examination with curvilinear array probe in supine position reveals hydronephrosis, complete loss of cortex, and inflammatory debris/sludge. Dirty shadows in the pelvis partially obscure the ureter.

Case Discussion

Pyonephrosis is the accumulation of pus in the kidneys with associated hydronephrosis. Patients clinically present with flank pain, fever, and vomiting. If not drained it can lead to complete kidney destruction and septic shock. Usually it develops secondary to some obstruction in the urinary pathway.
