Presentation

Patient Data

Age: 40 years
Gender: Female

Poorly defined hypodense involvement centered on the upper pole of the right kidney. Nodularity of the renal contour and cortical thinning are observed, associating deformity of the upper calyceal groups with urothelial enhancement. There are other patchy areas of similar cortical involvement, although less extensive and significant, in the meso-kidney and lower renal pole.

Dilation of the right excretory system, secondary to various areas of urothelial thickening with contrast hyperenhancement and periureteral fat striation that cause stenosis, the most significant at the level of the ureteropelvic transition and in the lower ureteral third, the latter more extensive with passage of filiform contrast in the excretory study.

Case Discussion

Renal biopsy reveals granulomas and the presence of Langerhans cells, confirming the suspicion of urinary TB.
