Presentation

Distended abdomen. Bowel obstruction?

Patient Data

Age: 65 years
Gender: Female
x-ray

Cholecystectomy clips.

Displaced gas in the left flank from a mass which contains linear calcification in the inferolateral aspect.

Benign calcific focus in the right upper quadrant.

Prominent small bowel in the central abdomen.

 

Case Discussion

Sure, abdominal radiographs purposes are not for the identification of masses, but it doesn't mean you can't identify one.

Here, mass effect is demonstrated on the small bowel and being in the flank a renal cause is high on the list of differentials.

On reviewing prior imaging a large Bosniak type II left renal cyst was identified as the cause of these plain radiograph appearances.
